About This Scheme
The scheme “Mukhyamantri Bal Hridaya Upchar Yojana” was launched by the Public Health and Family Welfare Department, Government of Madhya Pradesh. The scheme aims to provide free treatment and surgery to children aged 0 to 15 years who are suffering from identified heart diseases. The scheme provides financial assistance ranging from ₹25,000/- to ₹1,00,000/- for treatment, covering 07 identified heart diseases in children. Key features include covering costs for treatment at recognized government or private hospitals and ensuring the process is governed by the Madhya Pradesh Public Services Guarantee Act, 2010.

The beneficiary or hospital must submit a detailed expenditure report/utilization certificate for the funds spent on the patient’s treatment.
A duplicate or photocopy of the discharge ticket issued after the patient’s recovery must be provided.
Eligibility Criteria
The applicant must be a resident of Madhya Pradesh.
The applicant must be a member of an Economically Weaker Family or a Below Poverty Line (BPL) family.
The applicant must ensure their name is recorded in the family roster and they hold a ration card.
The intended beneficiary (child) must be aged 0 to 15 years.
The intended beneficiary (child) must be suffering from one of the identified heart diseases.

Step 2: In the registration page, provide the following mandatory information: Name, Gender, Mobile No., Email ID.
Step 3: Create a new password and confirm the same. Fill the captcha code. Verify your Mobile Number via OTP. Finally, click “Register”.

Step 2: Login using your registered Email ID and the password created during registration.

Grievance Redressal / Help & Support (Appeal Mechanism)
The applicant may file an appeal in the following situations:
Non-receipt of the application acknowledgment receipt.
Approved treatment amount is less than the prescribed package/estimate.
Non-resolution within the specified time limit.
First Appeal: Must be filed with the Collector. The prescribed time limit for the resolution of the first appeal is 15 working days.
Second Appeal: Must be filed with the Samagra Sanyukt Sanchalak, Swasthya Seva (Integrated Joint Director, Health Services).

Step 1: The interested applicant should take a printout of the
prescribed format
of the application form.
Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
Step 3: Submit the duly filled and signed application form along with the documents, within the prescribed period (if any), to the
Chief Medical and Health Officer (CMHO)
of their district.
