About This Scheme

The “Motor Vehicle Driver-Conductor Welfare Scheme- Gratuity Assistance in case of Death” scheme has been launched by the Transport Department, Government of Madhya Pradesh. The scheme aims to provide social security and welfare support to registered transport vehicle drivers and conductors. Under this scheme, gratuity assistance for funeral and last rites is provided to the family in case of death due to an accident. The scheme covers both the registered beneficiary and eligible family members.

Benefits

Gratuity Assistance in Case of Death

A one-time gratuity assistance of ₹2,000 is provided for funeral and last rites.

The assistance is payable in case of accidental death of the registered driver or conductor and the family members. .

Mode of Disbursement

The assistance amount is disbursed immediately by the local body.

The local body later claims reimbursement from the Social Justice Department.

Eligibility Criteria

The applicant should be a permanent resident of Madhya Pradesh.

The applicant should be a registered transport vehicle driver or conductor under the scheme.

The applicant should possess a valid commercial driving or conductor license issued in Madhya Pradesh.

The applicant’s name should be registered on the Samagra Portal.

The death should have occurred due to an accident.

The applicant should not be a government employee.

The applicant’s family member should fall under the defined

family category

Application Process

Apply Offline

Samagra ID Registration Process:

Step 1: Visit the official website:

samagra.gov.in

Step 2: Click on the option “Register Family” or “Register Member”.

Step 3: Enter the Aadhaar number and other required information.

Step 4: Enter the OTP received on the registered mobile number.

Step 5: Carefully fill in all the required details and upload the necessary documents.

Step 6: Enter the captcha code and click on “Submit Application.”

Offline Samagra ID Registration Process:

Step 1: Visit the nearest Lok Seva Kendra, MP Online Kiosk, or Common Service Center (CSC).

Step 2: Provide the required documents and necessary information to complete the registration process.

After Samagra Registration:

Step 1: Step 1: The interested applicant downloads the

application form

(Page No. 9) from the official website.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the District or Regional Transport Office.
